Please fill out one of the following. It must be signed by you and by the New School Administrator or Treasurer before reimbursement.

All staff who will receive a paycheck from the school will need to complete the following four forms before receiving a paycheck.
Please print these four forms, fill them out and bring them with you to your appoitnment with Tamara Breed. You will also need to bring two original forms of ID (no copies of ID, see the Federal I-9 instructions).
You cannot get paid without completing this process, so please be sure to attend to it promptly.
